While we accept most insurance plans, be aware that at times we are limited to certain plans. The restrictions change frequently. Call for information. Please be familiar with the type of insurance you carry to cover you and your family before you call. Bring your insurance card and a picture ID to every appointment and notify us of any changes. Currently we are unable to accept Medicade and Medicare.
All copays are due at the time of service. We accept cash, checks, Visa, Mastercard, and Discover.
